Obtaining Your Medical Records
If you have a new provider, please speak with your new provider to learn whether your past medical record is needed, and if so, for what period of time of care you received from us. We will provide a copy of your medical records directly to your new provider.
If you visited our practice since November 2020, you may download an electronic copy of your medical records by logging into your Athena patient portal.
If you have not visited our practice since November 2020 or you prefer for us to provide a paper copy of your medical records to you, please download and complete the Medical Record Request Form and email it [email protected].
Due to an unexpected demand for medical records, we ask for your patience in providing your medical record to you. We will acknowledge receipt of your request and will provide your medical record within 45 days of receiving your request.
